This example bill will define important areas of your billing statement

This is a type of insurance in which the insured pays a share of the payment made against a claim. For example, let’s say your shared costs are 20% of the “allowed amount” for a service. If the total cost of service is $100.00, you would pay $20.00, plus any unmet deductible, and your insurance plan would cover the rest of the “allowed amount” for service.

A co-payment is a fixed amount (for example $15) paid by the insured, typically at the time of service. The amount can vary depending on the type of health care service covered.

In-network healthcare facilities and/or providers are those who have contracted with your health insurance or plan. Co-insurance and co-payments are typically less with in-network healthcare facilities and/or providers than those who are out-of-network.

What does it mean to be preauthorized by my health insurer?

Pre-authorization is the process of obtaining approval by your health insurance plan for coverage of specific services, supplies, or drugs before they are provided to the insured. Make sure you check with your insurance to see if a pre-authorization is required, but remember that pre-authorization is not a guarantee that your insurance will cover the service.

All bills between clinics are combined for your convenience. Depending on your insurance however, lab tests ordered by your provider may be billed separately. You should also note that services provided at the hospital will result in a hospital bill that will be separate from your provider’s fees.

At Seasons Medical we offer three different options for paying your medical bills, and accept MasterCard, Visa, or Discover.
